Belviso SMAS Fusion Lift™ is not simply another device added to the clinic merely to extend the list of procedures. Its significance stems from a real clinical need: completing anti-ageing therapy conducted on multiple levels — from hyaluronic-acid-based products, through peptides, polynucleotides, CaHA, PLLA and PCL, to botulinum toxin, lifting threads and biostimulating procedures.

What is Belviso SMAS Fusion Lift™

Belviso SMAS Fusion Lift™ is a treatment platform using Micro/Macro-Focused Ultrasound technology, designated MMFU. The device delivers precisely focused ultrasound energy to selected tissue depths, without the need to breach the continuity of the epidermis.

The mechanism of action is based on creating thermal coagulation points, i.e. TCP. The ultrasound energy passes through the superficial layers of the skin and focuses at a defined depth, where it causes local heating of the tissues, contraction of collagen fibres and the initiation of reparative processes.

This is not a procedure aimed solely “at the wrinkle.” It is a technology directed towards tissue remodelling, improvement of firmness, lifting and biological regeneration of the skin. In practice, this means working on the causes of laxity, and not only on its superficial symptoms.

Microfocus and Macrofocus: two working modes of a single concept

One of the most important features of Belviso SMAS Fusion Lift™ is the ability to work in two modes of energy application: Micro-Focused and Macro-Focused.

The Micro-Focused Handle, i.e. microfocus, is used for very precise point-by-point and layered work. The ultrasound energy is concentrated at a selected point at a defined tissue depth. This mode is particularly important in anti-ageing procedures, where the aim is precise collagen stimulation, improvement of skin firmness and lifting of selected facial areas.

The Macro-Focused Handle, i.e. macrofocus, enables a more diffuse distribution of energy and even coverage of larger treatment areas. It is intended for volumetric and gliding work, the so-called sliding technique. It performs well in procedures to improve skin firmness, preventive procedures and therapies covering larger surfaces of the face and body.

The combination of both modes makes it possible to build therapy more flexibly. Microfocus is responsible for precision and layered work, while macrofocus handles the even working of larger fields and the filling of therapeutic spaces.

Biological mechanism: from the coagulation point to collagen remodelling

The action of Belviso SMAS Fusion Lift™ does not end at the moment the procedure is completed. The essence of the technology is the initiation of a controlled process of biological regeneration.

The first stage is the formation of TCP points. At the site where the ultrasound energy is focused, denaturation of matrix proteins, contraction of collagen fibres and the formation of microscopic coagulation foci in the tissue occur. Each pulse acts point-by-point, leaving the surrounding structures intact.

The second stage is the inflammatory phase, usually lasting from 1 to 7 days. The body begins a reparative reaction: macrophages are activated, fibroblasts are recruited and heat-shock proteins, including HSP47, are induced. This is a phase of biological preparation of the tissues for remodelling.

The third stage is neocollagenesis and neoelastogenesis, which develop over the following weeks. The presentation indicates a period of about 2–12 weeks as the time of increased fibroblast activity, synthesis of type I and III collagen and the formation of new elastin fibres. The process of collagen remodelling lasts about 90 days, which explains the gradual build-up of the treatment effect.

The fourth stage is tissue remodelling, which may develop over 3–6 months. It includes the shortening of collagen fibres, reorganisation of the extracellular matrix, an increase in tissue firmness and an improvement in skin biomechanics. For this reason, the effect of the procedure should not be assessed solely immediately after the procedure. The most important remodelling takes place over time.

Anatomical layers affected by the action

Belviso SMAS Fusion Lift™ acts on multiple levels. Depending on the chosen cartridge depth, the energy can be directed to the superficial layers of the dermis, the deep dermis, the subcutaneous tissue, the deep soft tissues and the SMAS layer.

The epidermis is not the main site of the device’s action. Its improvement is secondary in character and results from the remodelling of the deeper layers. In practice, this may translate into smoothing of the skin surface, better firmness, a subtle reduction in the visibility of fine lines and an improvement in overall skin quality.

The dermis is one of the main target layers. It contains fibroblasts, collagen fibres, elastin fibres and the ground substance of the matrix. The action of ultrasound on this layer is intended to lead to fibroblast stimulation, increased collagen synthesis, improved skin density and resilience, and reduction of fine wrinkles.

The deepest and key treatment level is the SMAS — the superficial musculoaponeurotic system. This is a supporting structure of the face, important for firmness, contour and the process of tissue descent. Working at the SMAS level makes it possible to achieve a lifting effect: contraction of the fibrous supporting structures, improved tissue elevation, better definition of the facial oval, reduction of descent of the lower face and improvement of the quality of the jawline and chin area.

Working depths and the significance of the cartridges

The system makes it possible to work with various depths, which allows the procedure to be adapted to the patient’s anatomy, skin thickness, treatment area and the aim of the therapy.

The 1.5 mm cartridge works in the superficial dermis and is used primarily to improve skin quality.

The 2.0 mm cartridge covers the dermis and supports improvement of skin firmness.

The 3.0 mm cartridge works in the deep dermis and is used for fine wrinkles, loss of resilience and deteriorated skin structure.

The 4.5 mm cartridge is responsible for work at the SMAS level and is key to the lifting effect.

The 6.0 mm cartridge works in the subcutaneous tissue and is intended for thicker tissues.

The 9.0 mm cartridge covers the deep soft tissues and can be used in contour therapy.

The 13.0 mm cartridge is intended mainly for work on the body and deeper tissue structures.

The choice of depth cannot be random. On the face and neck, a combination of several levels is most often used, in order to work simultaneously on the SMAS, the deep dermis and the superficial layers of the skin. In body procedures, greater depths are necessary, because the structure of the tissues, the thickness of the skin and the presence of fat tissue differ from those of the face.

Face and neck protocols

In face and neck therapy, a combination of several working depths is used. The standard protocol indicated in the presentation includes, among others, the M4.5 mm, M3.0 mm, M1.5 mm and D4.5 mm, D3.0 mm and D2.0 mm cartridges.

The clinical interpretation is logical: 4.5 mm acts at the SMAS level and is responsible for the lifting component, 3.0 mm works in the deep dermis and supports improvement of firmness and fine wrinkles, while 1.5 mm covers the superficial dermis and improvement of skin quality.

In the area of the nasolabial folds, the presentation indicates the use of the M3.0 mm, M1.5 mm, D3.0 mm and D2.0 mm cartridges. The aim of the procedure is to improve skin firmness and reduce the depth of the folds. This is not classic filling of the fold. It is work on firmness and tissue remodelling, which can complement but does not always replace injectable procedures.

In the eye and temple area, shallower depths are used, primarily M1.5 mm and D2.0 mm. The aim is to reduce fine wrinkles and improve skin firmness. This area requires particular caution because of the thin skin, delicate anatomical structures and the high visibility of any irregularities.

In the forehead area, the presentation indicates the use of M1.5 mm and D2.0 mm. The parameters are indicative in character and should be adapted to the thickness of the skin, the reactivity of the tissues, the patient’s age and the operator’s experience.

Body protocols

Belviso SMAS Fusion Lift™ can also be used in body therapies, particularly where the problem is laxity, loss of firmness and deterioration of skin quality. In body procedures, greater working depths are used than in the facial area.

For the arms, the presentation indicates the M9.0 mm cartridge as optional, as well as M6.0 mm, M4.5 mm, M3.0 mm, D4.5 mm and D3.0 mm. The aim is to improve skin firmness, reduce laxity and gradually remodel the tissues.

For the abdomen, deeper cartridges are used: M13.0 mm, M9.0 mm, M6.0 mm, M4.5 mm, M3.0 mm and D4.5 mm and D3.0 mm. The choice of depth depends on the thickness of the fat tissue and the degree of skin laxity. In this area, realistic therapy planning is particularly important. The technology can improve skin firmness and quality, but it does not replace surgical abdominoplasty in cases of a large excess of skin.

Collagen Cannon Handpiece and Ultrasonic Scalpel Handpiece

The Belviso SMAS Fusion Lift™ system uses two main types of applicator.

The Collagen Cannon Handpiece is a macrofocus-type handpiece. It is intended for procedures to improve skin firmness, maintenance therapies and procedures covering larger surfaces. It allows adjustment of the pulse frequency, energy power and treatment time. Its value lies in the even working of the area and the reduction of so-called empty treatment fields.

The Ultrasonic Scalpel Handpiece is a scalpel-type ultrasound handpiece, intended for procedures requiring greater precision and deeper action of the energy. The operator can adjust the length of the pulse lines, the spacing between coagulation points, the energy level and the working mode — single or automatic. This applicator is of particular importance where layered, precise work is needed in structures requiring a stronger lifting effect.

Interface and parameter control

The device’s control panel enables the operator to adjust the basic treatment parameters and to monitor the operation of the system. The most important functions include selection of the handpiece working mode, adjustment of the energy power, adjustment of the frequency, setting of the treatment time, control of the number of pulses and selection of single or continuous operation of the foot pedal.

The system also makes it possible to monitor the number of pulses delivered and the number of pulses remaining in the cartridge. From the standpoint of the safety and reproducibility of the procedure, this is an important element, because it allows control not only of the energy but also of the actual intensity of working a given area.

Significance in combined therapies

Belviso SMAS Fusion Lift™ is best understood as an element of a comprehensive strategy, rather than as a competitor to all other methods. Injectable products improve volume, skin quality, hydration, collagen stimulation or relaxation of facial expression. The ultrasound device, by contrast, acts on the architecture of the tissues and the deeper supporting structures.

In practice, it can be a logical complement to therapies based on biostimulators, fillers, polynucleotides, peptides, botulinum toxin and threads. It is particularly valuable when the patient does not require a further increase in volume, but rather an improvement in firmness, elevation and deep remodelling.

The correct sequence of therapy matters. Energy-based procedures such as MMFU are usually planned before injectable procedures, in order to avoid an undesirable effect of the energy on freshly injected products. The final scheme should be established individually, depending on the anatomy, previous procedures, the type of products and the aim of the therapy.

Patient profile

Belviso SMAS Fusion Lift™ may be considered in patients with loss of skin firmness, the onset of tissue descent, deterioration of the facial oval, weakening of the jawline, laxity of the chin area, loss of cheek firmness and deteriorated skin quality.

A particularly logical indication is patients who do not yet want surgical treatment but expect deeper stimulation than is possible with classic superficial therapies. The technology can also be used preventively in people whose aim is to slow the process of laxity and maintain tissue firmness.

On the body, the device can be used in areas of skin laxity of the arms, abdomen and other larger surfaces, with the proper choice of cartridges and realistic qualification. It should not be presented as a method for reducing large excesses of skin or as an alternative to surgery in cases of advanced tissue descent.

Safety and qualification

Belviso SMAS Fusion Lift™ is a non-invasive technology, but this does not mean that it is a trivial one. Working with ultrasound energy requires knowledge of anatomy, the proper choice of depth, control of the number of pulses, assessment of the thickness of the skin and subcutaneous tissue, and conscious planning of the protocol.

Proper qualification is of the greatest importance. The operator should assess skin quality, the degree of laxity, the amount of fat tissue, previous procedures, the presence of fillers or implants, general contraindications and the patient’s expectations.

The device has been designed for professionals in aesthetic medicine and cosmetology, but full use of its potential requires training, practice and verification of competence. High technology does not replace clinical judgement. Its effectiveness depends on the way it is used.
